About the Smile Giveaway Program
We understand that sometimes there are barriers between you and the dental care you deserve. The Smile Giveaway Program is designed to provide selected recipients with complimentary or reduced-cost dental implant and oral surgery treatments. Whether you are struggling with missing teeth or discomfort that affects your daily life, we want to hear your story.
Program Dates: March 30, 2026 – April 10, 2026
Application Deadline: April 10, 2026 at 11:59 PM MST
Who Should Apply?
We are looking for individuals who are ready to transform their oral health but are facing financial challenges. To be eligible for consideration, you must meet the following criteria:
- Age: You must be 18 years or older.
- Residency: You must be a legal resident of the United States and live within a 50-mile radius of our offices in Boise, Meridian, Hailey, Star, or Caldwell, Idaho.
- Financial Need: You must demonstrate a financial need for assistance.
- Commitment: You must be willing to follow the recommended treatment plan and attend all appointments.
How to Apply
Sharing your story is the first step toward a new smile. Our application process is simple, and we encourage you to be honest about your journey.
- Complete the Online Application: Fill out the application form between March 30 and April 10, 2026.
- Tell Your Story: Include a personal statement explaining why you need this care and how it would improve your quality of life.
- Submit Photos: Please include photos of your smile and any supporting materials requested.
- Submit by the Deadline: Ensure your application is submitted by April 10, 2026, at 11:59 PM MST. Late submissions cannot be considered.
Selection Process
Our team reviews every application with care. We select recipients based on clinical necessity, the potential impact on your quality of life, and our available resources.
We will notify the selected recipient(s) on or before April 30, 2026.
