Whitewater Dental Implants & Oral Surgery

Locations: Boise, Meridian, Hailey, Star, and Caldwell, Idaho

Program Period: March 30, 2026 – April 10, 2026

Application Deadline: April 10, 2026 at 11:59 PM MST

1. PROGRAM OVERVIEW

The “Smile Giveaway Program” is a promotional initiative to provide selected participants with complimentary or reduced-cost dental treatments at the sole discretion of Whitewater Dental Implants & Oral Surgery (“Sponsor”). Acceptance into the Program is not guaranteed, and awarded treatment plans may vary based on individual clinical needs, provider recommendations, and availability of resources.

2. ELIGIBILITY

Applicants must:

  • Be legal residents of the United States.
  • Be 18 years or older at the time of entry.
  • Reside within a 50-mile radius of Boise, Meridian, Hailey, Star, or Caldwell, Idaho.
  • Provide proof of residency upon request.
  • Demonstrate financial need.

Employees, contractors, affiliates of Whitewater Dental Implants & Oral Surgery and their immediate family members are not eligible. The Sponsor reserves the right to disqualify applicants who do not meet eligibility requirements or who have unresolved legal or medical matters.

3. HOW TO APPLY

To enter:

  • Submit a completed application via the Sponsor’s website by April 10, 2026 at 11:59 PM MST
  • Include any requested personal statements, photos, or supporting materials.
  • Agree to these Terms & Conditions and provide accurate contact information.
  • Provide documentation of financial need if requested.

Late or incomplete submissions will not be considered.

4. SELECTION PROCESS

Applications will be evaluated based on:

  • Clinical necessity.
  • Potential impact on quality of life.
  • Availability of clinical and financial resources.

Selected recipient(s) will be notified on or before April 30, 2026. All decisions are final.

5. PARTICIPATION REQUIREMENTS

Recipient(s) agree to:

  • Attend all scheduled consultations and treatment visits.
  • Comply with the recommended treatment plan.
  • Permit use of their likeness, photos, and testimonials for promotional purposes (see Section 9).
  • Follow all practice policies.

Participation in marketing, testimonial, photography, or promotional activities is completely voluntary and is not a condition of selection or receipt of treatment.

Noncompliance may result in disqualification.

6. RESPONSIBILITIES OF ENTRANT

(A) Representations

The Applicant confirms all submitted materials are accurate, original, and lawful.

(B) Rights Granted

Applicant may choose to voluntarily provide written authorization for the use of submitted materials and/or treatment-related imagery for marketing or educational purposes. Any such authorization will be obtained separately and is not required for participation in the Program.

(C) Release

Applicant agrees to release and indemnify the Sponsor from any claims arising from participation.

7. LIMITATION OF LIABILITY

The Sponsor is not responsible for technical failures, interruptions, or unauthorized interference. Any attempt to compromise the Program may result in disqualification.

8. PRIVACY

All personal information will be handled in accordance with HIPAA and applicable privacy regulations.

9. PUBLICITY RIGHTS

Participation in publicity, including the use of name, image, voice, or likeness, is voluntary. Separate written authorization will be obtained prior to any such use, and participants may decline or withdraw consent at any time without affecting their eligibility for or receipt of treatment.

10. RELEASE OF CLAIMS

Applicants release the Sponsor and Program Entities from all claims associated with participation.

11. DISPUTE RESOLUTION

All disputes shall be governed by and resolved in the courts of the State of Idaho, without class action.

12. GENERAL

All federal, state, and local laws apply. The Program is void where prohibited.
